Coordinator- DH (Project Management)
The Day Hab Coordinator is responsible for all aspects of the Day Habilitation Program to provide the individuals supported the opportunity to increase independent living, socialization and vocational skills through participation in person centered planned activities in the community and at the program site.
Essential Duties and Responsibilities: include the following.
Supervision of the people we support Implementation of Staff Action Plans Assist in planning of activities Assist the people we support when needed including areas of self care Transportation/Driving Vans Attend all mandatory training sessions including Orientation, SCIP, First Aid and CPR. Complete all necessary documentation including: Staff Action plan goals and supports ,Data Collection and Activities Record on a daily basis; Day Hab Monthly Summaries including distribution; Day Hab Assessments; Medication Records (if AMAP); Incident Reports, Seizure Reports and Behavioral Records when needed; and necessary correspondence. Attend all necessary in-service trainings including First Aid, CPR, Orientation, AMAP and SCIP as needed. Supervises the Direct Support Professionals in the completion of their job responsibilities outlined above Assist the Direct Support Professionals in completion of required Agency Paperwork including attendance, transportation and Human Resources records. Any additional responsibilities for the successful operation of the program.
Supervisory Responsibilities:
Directly supervises 10-15 employees in the Day Habilitation. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include assisting the Manager/conducting in interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
