Outreach and Intake Coordinator (Personal Services)
Serves as the initial point of contact for families about YCS programs.Responds to program inquiries/referrals within two business days of inquiry/referral.Works closely with program teams to ensure smooth transition of referrals and to problem solve when issues arise.Attend team meetings; including but not limited to bi-weekly meetings, outreach meetings, and ongoing supervision meetings.Complete initial enrollment forms and assign to appropriate program during triage.Advocates for children, teens, adults and their families by working with community collaborators to assist families with their needs and services.Maintains accurate and organized records of assigned cases and referral outcomes.Communicates with other agencies to forward referrals outside of programming service area.Responds to all inquiries and referral sources within two business days to verify referral receipt and to provide a timeline for services as well as other program options.Sorts and organizes referrals by zip code for distribution to programs.
3. Manages and develops strong community relationships in order to promote programming.
Works in collaboration with YCS teams to develop and maintain a robust network of resources and collaborative relationships.Maintains a spreadsheet of outreach activities, including community events, workshops, appearances, and other opportunities.Nurture new and ongoing relationships with collaborative partners.Coordinates and attends workshops and informational sessions for families and professionals.Attends, facilitates, organizes, and conducts community-based activities in conjunction with the YCS team.Communicate effectively with other social service agencies to assist with referrals and providing information to the participants regarding other available services located within the community.
4. Meet the varying needs of diverse families.
Accommodates families with limited English and/or other differently abled needs so they may easily access YCS programs.Advocates for families within to ensure we are serving and meeting the needs of families with limited English proficiency, non-English speaking, and/or families with differing abilities.Collaborate with YCS staff to ensure events/programs offered meet the needs of enrolled families.Ensure programs are accessible and marketed to families enrolled in programming.
