Social Services Director (Personal Services)
We are seeking a dynamic and experienced Social Services Director to ensure residents and families are assisted with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the duties of the position.
SUPERVISORY RESPONSIBILITIES:
Directly supervises employees, ensuring compliance with facility policies and applicable laws. Responsibilities include interviewing, hiring, and training staff; assigning and directing work; evaluating performance; managing rewards and discipline; and resolving complaints. Must exercise discretion and independent judgment consistently.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Resident Evaluation and Treatment: Regularly evaluate residents' social and mental health needs, collaborate with the interdisciplinary team to develop and review care plans, and communicate resident concerns, preferences, and needs.Admissions and Discharge: Assist with admission, transfer, and discharge processes, including mental health evaluations, care planning, discharge summaries, and post-discharge care coordination.Family Communication: Regularly communicate with residents and families about social services, mental health, and resident preferences, and promote activities to enhance social well-being.
Documentation: Ensure proper documentation, including social services assessments, RAI/MDS updates, and progress notes.
Quality Assurance: Perform regulatory compliance rounds, observe social services provision, review records, and implement corrective actions as needed.
Policy Development: Assist in developing and updating social services policies and procedures to align with facility standards and legal requirements.
Survey Participation: Support the Executive Director in the survey process by instructing staff, being present during surveys, and ensuring compliance with survey requests.
Continuing Education: Maintain up-to-date knowledge through ongoing education and in-service programs.
Compliance: Ensure responsibilities comply with all federal, state, and local laws, regulations, and facility policies.
Emergency Response: Ensure resident and employee safety during emergencies.
Skills and Abilities: Possess strong interpersonal, organizational, communication, and documentation skills, with a high level of energy and enthusiasm.
Physical Requirements: Requires physical activities such as lifting (up to 25 pounds), standing, sitting, walking, and exposure to medical equipment and potentially ill persons.
EDUCATION and/or EXPERIENCE:
High school diploma or general education degree (GED) (Required).Bachelor's degree (Preferred)Two to four years related experience and/or training or equivalent combination of education and experience.Previous social services experience in a health care setting preferred. Meets state and federal requirements.Social Services Designee Certificate (SSD) (Preferred)
CERTIFICATES, LICENSES, REGISTRATIONS:
Driver's license per facility requirements preferred.
